Forms — Core Walkthrough

Accessing the Forms Builder
Start here
From your AI Boost Solutions dashboard, navigate to Sites in the left sidebar.
Click on the Forms tab at the top of the page.
You'll land on the Forms Library — this lists all existing forms in your account.
Click + New Form (top-right) to open the drag-and-drop builder.
💡 Tip: Forms are ideal for single-page data capture — lead gen, contact forms, opt-ins, and appointment bookings.

Adding & Configuring Fields
Builder
Use the left panel to drag field types onto your form canvas.
Click any field to open its settings: label, placeholder text, required toggle, and custom name.
Reorder fields by dragging the handle on the left side of each field block.
Use Custom Name to map fields directly to contact custom values in AI Boost Solutions.

Form Styling & Branding
Design
Click the Styles tab in the right panel to customize colors, fonts, button text, and background.
Set form width, field border radius, and label font size to match your brand guidelines.
Toggle Show Branding off for a clean, white-label appearance on client-facing pages.
Use the Custom CSS box for advanced styling overrides beyond the visual editor.
💡 Tip: Style settings apply per form. Duplicate a styled form to reuse it as a branded template across multiple campaigns.

Submission Actions & Notifications
Automation
Go to the Options tab in the builder.
Under On Submit, choose: show a thank-you message, redirect to a URL, or redirect to another page in your account.
Enable Send Notification to email your team or the contact automatically on each submission.
Use merge tags like {{contact.first_name}} inside notification emails to personalize messages.
Turn on Create Contact to automatically add form submitters to your CRM.

Surveys — Multi-step Walkthrough

Creating a New Survey
Start here
Go to Sites → Surveys from the left sidebar of AI Boost Solutions.
Click + New Survey — surveys are multi-step by design, perfect for onboarding flows, quizzes, or client intake.
Each "slide" represents one page the respondent sees at a time.
Click + Add Slide in the left panel to expand your survey with additional steps.
💡 Tip: Use surveys instead of forms when you need to break long intake processes into digestible steps or require conditional question branching.

Configuring Slides & Question Types
Builder
Each slide can hold one or more fields — drag them from the left panel, same as the forms builder.
Add a slide title and description at the top of each slide for context and respondent clarity.
Survey-exclusive fields include: Star Rating, NPS Score, and Scale inputs.
Rename each slide in the left panel for easier navigation during editing sessions.
💡 Tip: Keep each slide focused on one topic. Overloading a single slide significantly reduces survey completion rates.

Conditional Logic Between Slides
Logic
Select any slide and click Add Condition to define which slide appears next based on a field answer.
Example: If Service Interest = SEO, jump to the SEO-specific slide. Otherwise, continue to the default next slide.
Conditions are evaluated top-to-bottom — drag to reorder them by priority.
Always set a Default Next Slide as a fallback when no condition is matched.
💡 Tip: Logic branches let you deliver a personalized experience to different lead types without building separate surveys for each segment.

GDPR, Spam Protection & Submission Limits
Compliance
Enable reCAPTCHA under Options to block automated spam submissions.
Add a Consent Checkbox field for GDPR compliance — it logs the consent timestamp directly to the contact record.
Set a Submission Limit per contact or per IP to prevent duplicate entries.
Toggle Limit to One Response for surveys where each contact should only respond once.

Connecting to Workflows & Pipelines
Automation
Inside Workflows, use the trigger Form Submitted or Survey Submitted to fire automations on each submission.
Filter by specific form or survey name to scope the trigger to one form only — essential when running multiple forms in the account.
Use the Add to Pipeline action to move contacts into a pipeline stage automatically after submission.
From the same workflow, add tags, update custom fields, and send follow-up emails or SMS sequences.
💡 Tip: Survey responses populate contact custom values — use those as workflow conditions for hyper-targeted, segmented follow-up sequences.

Using Form Data in Automations
Advanced
All form and survey field values are accessible as {{custom_values.*}} inside workflows, emails, and SMS templates.
Use If/Else branches inside a workflow to route contacts differently based on their form selections.
Combine with Wait steps and Goal Events for drip sequences timed to form submission behavior.
💡 Tip: Name your form fields consistently across all forms to make workflow conditions and merge tags easier to manage at scale.
Embedding & Sharing Your Form or Survey

Embed Code & Direct Share Link
Publish
Click Integrate → Embed Code at the top of the builder to access publish options.
Choose between Inline Embed (renders inside a page), Full Page (hosted standalone URL), or Popup Trigger (button-activated overlay).
Copy the snippet and paste it into any website builder, funnel page, or external HTML site.
The Share Link tab gives you a hosted URL to drop directly into emails, SMS campaigns, or social posts.
💡 Tip: Use the popup embed on landing pages — trigger the form only when a visitor clicks a CTA button to reduce friction and increase opt-in rates.

Viewing Submissions & Analytics
Reporting
On the Forms or Surveys library page, click the three-dot menu on any form → View Submissions.
Filter submissions by date range, contact name, or submission status.
Export all submissions as a .CSV file for external reporting, analysis, or client delivery.
Survey analytics show per-slide drop-off rates — identify exactly where respondents abandon the flow.
Every submission also appears on the contact's record under the Activity tab — no need to leave the CRM.
💡 Tip: Review per-slide drop-off on surveys monthly. A high drop rate on a specific slide signals it needs to be simplified or repositioned in the flow.
